£50 Cash Back Promotion
Terms and Conditions
1. OCUPLAN CASHBACK INITIATIVE OVERVIEW
1.1 To enter the cashback promotion, the Participant must purchase a Product from their OcuPlan Opticians during the Promotional Period AND join OcuPlan by telephone between
1.2. All Participants must be aged 18 or over and must be resident in the UK.
1.3. All Participants are deemed to have agreed to be bound by these terms and conditions.
2. HOW TO PARTICIPATE
2.1 To qualify for the Cash Back Initiative, Participants must sign up to OcuPlan after purchasing their eye care product from their OcuPlan Optician. The participant must quote the code on the voucher.
2.2 To sign up to OcuPlan under the Initiative, Participants must call the

3. CASH BACK
3.1. Participants will receive Cash Back to the total value of £50.
3.2. Upon signing up to OcuPlan with a valid promotion code, OcuPlan will provide a cheque for the total amount included with the Participant’s welcome letter.
3.3. OcuPlan will endeavour to provide the cheque within 5 working days of the Participant joining OcuPlan.
4. THE PROMOTER
4.1. The promoter is OcuPlan Limited, company number 12293810 with its registered office address at Hambleden House, Waterloo Court, Andover, England, SP10 1LQ.
5. DISCLAIMERS
5.1. In exceptional circumstances, OcuPlan may void, amend, and/or change this promotion at any time without incurring any liability whatsoever provided that such changes are necessary and can be justified by objective reasons.
5.2. OcuPlan reserves the right to audit any claims to ensure compliance with these terms and conditions and to request additional information and supporting documents. OcuPlan reserves the right to exclude claims and/or Participants if it suspects that the promotion is being abused in any way. OcuPlan’s decisions in relation to the promotion are final and no correspondence will be entered into.
6. CANCELLATION
6.1. If Participants wish to cancel their OcuPlan membership, they will be able to do so after three months from the date of sign up. If at this point no visits have been used, a full refund will be provided minus £50.
7. DATA
7.1. OcuPlan Limited is the Data Controller in respect of any personal data supplied by Participants. Any personal data submitted to OcuPlan Limited will be processed and managed by OcuPlan Limited in cases where Participants have opted-in to receive marketing communication.
